Depends on where the rag was. In an air box/intake manifold on a big diesel, it'll just get sucked into a cylinder, pulverized, and turned into ash. However, if it has been stuffed into an oil or water passage and is completely occluding it, that's a different story. You'll be walking around the front of an engine and be like, "Huh, that's a weird noise coming from the timing gears", and then, that night, you and the boys will pull the cover off and see a rag completely stuffed into an oil passage, starving key components of oil. The timing gear will be missing chunks of gear teeth, which will later be found in the oil.
