Without being shit-faced Nick has no idea what to do on stream anymore. He looks super uncomfortable and keeps going off on nonsense tangents. This is more coherent than his normal drinking streams, but he really is a shadow of his former self. I think Josh is right that he's suffered permanent brain damage.
