High blood pressure. Ragepig sustains himself in a steady diet of tortas and xannies with 0 physical activity in his life. It's only a matter of time till he has heart failure and a Medical doctor has to do a bypass.

Luckily, I think they train doctors with pig hearts, so it won't be totally unfamiliar. Unluckily for Ralph, he's going to wake up on a ventilator after surgery completely alone in an unfamiliar room with no one giving a damn. This is his certain future.
