If Russia (or the USA) fires off all its nukes, it doesn't matter anymore. It's over. The only meaningful defense against that is preventing total nuclear war from happening in the first place. My interpretation of the new policy paper is the US reminding the Russians that we, too, have functioning nukes and they should maybe stop threatening to use theirs as a hissy fit for not winning the Ukraine War. It's basically saying "hey, quit being a retard."

If this was the USA at the their height of power, unified and strong and top of their game in industry and logistics, maybe one small (200kt) nuke on a single city could be tolerable.

It is not. A single Hiroshima firecracker alone would overwhelm most of the USA. Even during the 1950's when the USA only had to contend with a few dozen soviet nukes the studies done showed that even with some storage there wouldn't be enough medical supplies to help a single large city in the aftermath of a attack much less the entire nation.

You are beyond delusional if you think that a nuclear war is something that can be just shrugged off like that. Take a fucking step back and look at the damage a goddamn jumped up flu did to the USA and the strain it put on the economy, society and government.
