Family member, friend? Is there someone who would write you a letter of recommendation if that would help? Perhaps your GP or another health worker?

I may be very wrong, but such experience as I have suggests to me they are looking for some sort of informal reassurance that you will be able to manage the course - in terms of its social and emotional demands, not its intellectual ones - from some sort of 'third party'.

A letter of recommendation might achieve that. If you were in a position to offer one, i think they would also find that reassuring. Of course, you would need to approach whoever you wanted to write the letter before offering to provide it.
