Hi all! Back again with another, likely final post!
I am nearly 2 full years post-op Penile Inversion Vaginoplasty with Dr. Laungani of Atelier Du Corps, formerly with GRS Montreal when I had my surgery done. I am also nearly 5 months post-op Breast Augmentation with Dr. Genoway in Vancouver.
I could not be happier with both procedures!
Laungani did a fantastic job on my bottom surgery and I don't have any wishes for revision nor any lasting complications.
Genoway also did a fantastic job, she recommended 385CC Natrelle Inspira SSF (Soft touch, Smooth, Full Profile) implants for each side and I love the size in consideration with my frame. I ended up with about a 34D-DD from an AA-A on a good day. She recommended dual plane, with the upper half of the implant under the muscle and the bottom half "floating over" the muscle, effectively an open pocket, I love the natural shape and movement as well as hold that this option provided!
Both of these procedures were covered by Canadian Provincial Healthcare and I could not be more grateful for the opportunity to get them done.
These procedures have allowed me to move through the world as a woman without question, I met my darling fiancé four years ago at an old job we worked for together and have been with him nearly a year, I have my own apartment, a new job and more opportunities, found myself a mental health care team and proper medications, life could not be better right now.

So I'm 16 months post op and still have very big scars. My surgeon says that it's normal and if I want a revision, I would have to pay. So I'm wondering if I should invest more in some kind of scar treatment with a dermatologist or just straight up go for a surgery.

I haven’t done a post in several months and figured I was long overdue for one. It’s worth noting that while I’m 10 months post op from stage 2, I’m a year and a month post op from stage 1, and approaching 5 months post op from the urethroplasty. I’ll be doing a separate post that’ll have photos of my arm specifically.
Now, my last post regarding my dick was when I was 2 months post op (stage 2) so I’m going to be talking about how things have gone from there. I had mentioned in this post that my stream had narrowed a bit but it was nothing to a concerning level or causing problems. However, this changed once I reached 3-4 months post op. My stream had narrowed even further which had lead to a slower stream and would often times take a sharp left directly from the tip. It wouldn’t always do the sharp left so I couldn’t just preemptively adjust my penis to aim in the appropriate direction otherwise I’d end up pissing elsewhere. I reached out through MyChart and Zhao’s (UL surgeon) team had reached back out stating that it could be possibly due to granulation tissue but that if it was, they could remove it during a cystoscopy. They then proceeded to schedule me for one with Zhao which actually happens to be in the same building as Bluebond just on a different floor. A quick side note— there are other urologists on this floor so the people going there are not strictly phallo patients. Anyways, luckily enough, it turned out that it was just granulation tissue and they were able to remove it! The cystoscopy and removal weren’t painful for me whatsoever. I peed directly after the appointment and it was an immediate relief. No more narrowness and no more sharp lefts. Unfortunately, within the span of 2 weeks my stream had progressively narrowed yet again and this time worse. I could feel pressure build up as I wasn’t able to urinate at a normal rate which would lead to discomfort and some slight pain. I reached out to the team to update them and got scheduled for another cystoscopy. This time, it appears that I had a meatal stricture which they were able to dilate. The dilation didn’t hurt but they did also use a lidocaine jelly before doing so. I had a bit more internal sensation this time around so I was able to feel the cystoscope a bit more. It was an odd sensation and wasn’t painful until it started nearing the bladder and entered it. I got so lightheaded from it that I thought I was going to pass out. To be fair though, I also did not have anything to eat or drink before the cystoscopy this time around which I’m sure didn’t help with the lightheadedness. I was also warned that I may experience this pain, discomfort, and/or lightheadedness before they started nearing the bladder as it’s very common so I wasn’t completely caught off guard. Again, I peed directly after the appointment and it was immediate relief. But alas, this was short lived. Yet again, all my symptoms returned and had become even worse to the point it would take up to 3 minutes to pee at a time. I reached out once more and was scheduled a visit where a nurse would show me how to dilate as Zhao was unavailable. During this appointment, the nurse was unable to insert the standard size and had to work their way down. They were only able to use the smallest size they had and were unable to increase the dilation from there as even that size was met with some resistance. I was given some to take home and had to wait until I can get seen by Zhao for another cystoscopy. Unfortunately, no improvement was made and after several attempts, he couldn’t get the cystoscope through. Thus, the decision was made that I would require a urethroplasty. It was noted that I had a “nearly obliterated fossa navivularis” and it was labeled as a meatal stenosis. Luckily, I was able to get surgery a little over a week after this appointment. Since it was so last minute and urgent, I was kind of squeezed into the day. So I did spend more time waiting in the hospital than in surgery as surgery was fairly short. I don’t say this as a complaint but just worth noting in case anyone else has to go through this. I think I spent 4hrs before getting taken into the OR and surgery was probably about an hour at most. A vertical incision was made so the opening was only slightly lowered but the overall appearance hadn’t changed much. I followed up with Zhao over a phone call about a week and half after.
Since then I’ve had two follow up visits with Bluebond’s team. To put a timeline, I had the urethroplasty at the end of October, a follow up with Bluebond’s team in November, and another follow up just last month. I’ve been healing great and fortunately have had no further issues. However, at my last follow up they did say they would like to still wait until my next follow up in May before going ahead with stage 3. It’s a bit disappointing but I understand the precautions and ultimately I’m grateful to be where I am.
Other than that, everything has been going well. I believe all the swelling has gone down now. My length has stayed at 5.5” but can measure up to 6” if I’m slightly tugging on it. My girth has gone down by about 1/2”. The base measures about 5” while the rest is slightly over 4.5”. With the swelling all down, my dick is a lot softer overall as is the scrotum. As you’ll see in the side profile photos, my dick doesn’t protrude as much due to this which I’m sure will change once I get implants + ed. I am currently having mixed feelings about my glans and am contemplating on a possible revision as it seems some flattening has occurred. I’ve been waiting for all the swelling to go down since I had noticed one side of the underside of glans is slightly more raised and while it has improved after all the swelling has gone down, it’s still noticeable. This is more so just something noticeable to me since it would have to be at certain angles. I also feel the underside of the glans has a slight “wrinkle” look. Now, it’s not like I hate how my glans look but I do have moments where all of this starts to accumulate and has me feeling like it doesn’t look “natural enough”. This isn’t a constant feeling though and there are many moments that I’m happy with how everything looks and I know I’ll feel that even more once I have tattooing. At this point though, I care more about moving onto stage 3. I may reach out to ask if a revision can be done during stage 3 and if I can, then I will. Otherwise, I rather have it done after. Aside from that, there is also a very slight overhang of the taint above my butthole. This doesn’t necessarily bother me but I’ll bring it up just in case it’s anything that can somehow affect milking and if it’s even worth addressing as I do still experience some leakage.

The wound dehiscence is actually quite serious. My urologist says the opening in the perineum goes all the way to the rectum. The wound above the new urethra has also gotten worse rather than better. My urologist wanted to send me back to the clinic where I had the surgery. But that would have meant a 7-hour train ride each way, and I really wasn’t feeling well enough at the time. The surgical clinic told me to just wait and that it would heal on its own. In the end, my GP arranged outpatient wound care for me. A wound care specialist came up with a treatment plan, and I’m already feeling much better. I really hope the urethra stays intact. The Foley catheter should definitely stay in for now. Other than that, this will take a lot of patience. The main thing is that it heals.
