As for surgical treatment? First off, he'd have to let his rotten crotch wound close by itself, and help the healing process with medication, if when necessary. Anti-inflammatories, proper wound dressing, frequent cleaning.
After removing the stink part of the stink ditch and minimizing the risk of actual rot and ingrowing hair, obviously, but with the 'rona, well, there's no chance it could be done any time soon.

Then a urologist surgeon would have to re-route his urethra again, if it's even possible. Then more surgeries for his pelvic floor, again, if it's possible to fix it up somehow. Not in the sense you are referring to. The rotten flesh would most likely need to be excised and the entire area debrided. From there, the wound can be sutured closed, preferably with a drain so the suppuration won't build up and force the wound open again.
Provided he changed the dressings regularly and left it the fuck alone for fucks sake, it should heal up and the drain can be removed. He'd still probably have to piss through a tube for the rest of his life, but considering the other option is sepsis from a rotting wound, I know which one I would choose if forced to.

Most of the names on their latest GoFundMe are troons or orbiters. 50% who donated 100+ are anon (or in one case have an initial as last name- anon imo) which I imagine is odd for the platform?

When looking into their GoFundMe campagins I only found 3, thought they had more?
But what's weird is the one intended to fund the move was started in 2018.
