I've actually seen better prison tattoos than that. Specifically, one dude who had facial tattoos. Chill guy, but nobody fucked with him. Extensive facial tattoos basically advertise you have a pain tolerance through the roof.

But several problems comorbid with autism explain it...

Prosopagnosia (Face Blindness) shows us that autistic people can't tell faces apart easily. Remember, Chris thought the pictures of four different women were all "Blanca Weiss". Phil might not be able to distinguish his face from any other round, light-skinned face. He doesn't understand what would look "feminine" to begin with.

Lack of theory of mind would mean that Phil doesn't comprehend what's "obvious" to him isn't obvious to anyone else. He thinks he looks more feminine -he even tattooed his skin to indicate he was not some cis man!- and that should be obvious to you.
