These designs have pretty bad same-face syndrome, a style too simplistic for its own good and could easily pass for Steven Universe characters. The characters frankly look like poorly-done caricatures to me. For further information, these 2 posts can be of use.

They're the left behind husks from things that have lost their hearts, I don't think they're meant to look monstrous per se. More like vaguely humanoid with something very off about them.

But in the late 80's early 90's AIDS was a huge epidemic, it was somewhat important for older kids to be aware of it and captain planet was a PSA show already
To be aware of it and also aware that many rumors about HIV/AIDS were not true. Many people really did believe that you could get HIV through handshakes, or sneezes. The Captain Planet episode only aired two years after the death of Ryan White, who had become a poster boy for AIDS when his hometown blocked his attempts to return to school, where many parents withdrew their kids when he did return because they believed he was highly contagious. So it was very important to make public awareness to the fact that HIV is not transmitted through casual contact, which was also the reasons for why South African Sesame Street eventually added an HIV+ character.
