It's possible Barb told the police what was happening while she was in the hospital though, or they found enough evidence they wanted to press charges. You can be forced by police to go to the hospital based on internet rumors.

This is what probably happened. Barb may not have been aware that incest is illegal among adults, assuming that it really was consensual. She may have figured that if she tells them that it occurred but that she was OK with it, all of this would disappear.
