She has severe rheumatoid arthritis. That's why she is fat, she can't do much physically without intense pain. She could have gotten gastric bypass or something to maintain appearances I guess, but she is already suffering a lot so I don't blame her for avoiding surgery. She has done some A+ voice acting since she got sick.
I was on Netflix and an ad for The Umbrella Academy was at the top of the page. I lol'ed at how much smoothing they had to do on Ellen Page compared to how she looked when talking about Smollett. They kept the 5-head though.

She looks emaciated though, IMO. She looks like she's nothing more than a skeleton wearing Ellen Page's skin. Like, I don't believe that there's any blood, muscle, or fat under that skin at all. I'm not commenting on whether she's anorexic or not ... I'm just commenting that, to me, she does indeed look like the crypt keeper wearing an Ellen Page costume.

I was once on a tv show, and they put standard makeup on me and everyone else. After we were finished, the director wanted a "reaction shot" from me which wasn't caught effectively, so I was basically shoved into place and had to look sad for a a few seconds. What no-one at the time noticed was I'd washed off the makeup, so for about three seconds I look a lot worse (mildly spotty, dark around the eyes) than the rest of the episode.
It made me feel GREAT when I was following along with the hashtag and about three people were like "urgh what happened to him there"
I bet the network LOVED that, as it was all meant to be real!
So yeah, camera + tv lighting really does need at least a token layer of makeup, I'm willing to bet Ellen Page looks at least a bit less ghastly in person.
Drugs, booze, smoking, bad plastic surgery and an ill advised career as a boxer (though he's technically undefeated, suffering 2 draws) didn't do him favors.

It's the lines on her neck. You never expect to see those on anyone not >40, or in atrociously bad shape of some sort. (Ya Boi Zack noted, and I think this is a truism among artists, that every line you add to the face makes it look older. Ellen has a lot more lines than you expect on someone of her notional age.)
Shelley Duvall was never anyone's idea of drop-dead gorgeous, but I can at least acquiesce that she might have been seen as "cute" back in the day. She did not enter her golden years gracefully, I'm sure her mental illnesses has a lot to do with her appearance, but she looks almost unrecognizable now.
