If she doesn't want to end up in the hospital with aspiration pneumonia, Alex is going to stop drinking at some point. Her swallowing issues have already been documented, but alcohol (a depressant) is absolutely not helping her with those issues. She likely thinks that she's too good for thickeners for liquids, ofc. I wonder how a claw would taste with liquid thickener in it.
 
Considering dysphagia is a known issue for SMA it is odd she has never addressed it. But if someone becomes incapable of swallowing their own saliva they do have surgeries to remove the salivary glands.
You can also start with simple things like atropine drops (used for hypersalivation that occurs with medications like clozapine), but she would need to be really on top of her mouth hygiene.

Alex likes to portray herself as this bad-ass disabled warrior who gives zero fucks and can do anything an abled person can do. Look at me, everybody! I can drink and party and live "independently" and have unprotected sex with losers and have a high risk pregnancy that produces a premature baby, haters!
In reality, if you left her by the side of the road with her wheelchair battery drained, she'd be dead in 48 hours.
It's like... okay, I get it. It fucking sucks that she's so profoundly disabled and is completely dependant on others for literally every single thing but there comes a time where you have to quit acting out and accept the facts of your shitty life and stop railing against how unfair everything is.
With Alex for a mom, Ari's life is going to be difficult. Even with Noah's parents' support, Ari will struggle simply because Alex is her mother. Ari's existence is the result of Alex trying to pwn the haters and how utterly fucked up is that?
And now, what is going to become of them? Alex's future isn't exactly bright; especially considering the degenerative nature of SMA. She's going to become even more disabled. How will she support herself and Ari? Alex has mentioned that she's "saving money" (while buying the latest iphone) and, with the admission that she and Noah are no longer a couple (doubtful), plus her recent co-opting of the "single mom" tag, is she seriously planning to just take Ari and leave when she's ready? Good luck with that. I think Noah's parents will only put up with so much of Alex's bullshit before something life-altering happens. Since Alex won't address her issues and just wants to party and be a bad ass, the kindest thing she could do for Ari would be to just peace out and see her a few times a month. Alex can be the fun mom! She can take Ari out for lunch or for a mani-pedi or for the occasional shopping spree. Ari needs stability and because Alex won't actually admit how shitty it is to be so profoundly disabled and get some goddamned therapy for it, Ari will pay the ultimate price.
 
There is a moment in the live where she bitches about the cost of baby formula and when someone asks her about WIC (a federal program to help mothers of young children purchase formula and food) she obviously has no clue what that is or whether she actually qualifies for it. While living with Noah’s parents probably does put her over the household income threshold in Florida, she would still qualify if she receives Medicaid, regardless of income. If she has SSI, she is entitled to Medicaid in Florida. I cannot envision any scenario where she does not qualify for SSI and therefore Medicaid and therefore WIC…unless she does not have legal custody and this somehow changes the math?

Point is, I know what WIC is and I am neither a poor nor a parent, This dumb bitch has nothing to do all day, the least she could do is some fucking research into bettering her situation for her daughter and the people raising her. And see a doctor about her fucked up swallowing.

They kept pointing out that you can literally buy that brand of formula (she said she used Enfamil Gentlease) for like 20 bucks. Its even cheaper per ounce if you buy the jumbo sized, double packs from Sams/Costco.

Swallowing seems to be a deliberate, and very difficult, conscious effort for her.

We all swallow instinctively as we sleep.

So does she just not swallow at night, and then wake up in a gargantuan puddle of drool each morning?

She does clearly use the nose hose kind of CPAP rather than the full mask kind - maybe that allows her to leak as need be.

Which reminds me that one of those Reddit screenshots posted above refers to Alex's chair towel as her "tuna towel" :story: Gross

Another interesting post from Reddit was that other person with SMA who said that she should really use a bipap rather than a CPAP, which makes sense considering how SMA works - and also begs the question - if her muscles failed while she slept would the CPAP just inflate her like a balloon?

You have to have a respiratory drive for CPAP/BiPAP to work (also fwiw, those are both actually trademarked terms, and you may see NIV [non-invasive ventilation] and bi-level NIV used as those are the "generic" terms).

They both just give you a boost to your breathing, but CPAP applies a consistent back pressure during inspiration and expiration whereas BiPAP senses when you take a breath in (because the pressure in the circuit drops) and gives a higher boost of pressure as you breathe in. Then as you breathe out, drops back to the background pressure.

My experience is only with people who are unwell, and in that case you never use CPAP because you're aiming to assist them taking a breath AND provide some background back-pressure to help keep their small airways lower down in their lungs open. CPAP in the home setting is pretty much just to provide back pressure to keep your glottis open so you don't snore/stop breathing because you're (probably) fat with a short neck.

Anyway, for someone like Alex that has a structural reason for having shit breathing (muscles of her diaphragm and chest wall that can't help her take a deep breath when asleep), it would make sense to be using a BiPAP setting especially at night. I mean it makes no real difference to the user, but they do need to have a machine that can do bi-level pressures.
She should also be seeing a Sleep Physician at least once a year to check the data of the machine and adjust the settings as needed. The settings she started on are unlikely to be relevant when she has a progressive disease.

Also she wouldn't blow up like a balloon, simply because she has a nasal mask so if her mouth opens a percentage of pressure will just come out her mouth (for that reason you can't use nasal masks for mouth breathers).

I'm not sure why Noah's parents wouldn't ensure Alex and the baby aren't on every program for which they're eligible in order to reduce the parents' considerable financial burden of caring for 3 people who don't/can't contribute. Very curious.
I wonder if this is deliberate. Maybe setting up a future attempt to gain legal custody of the bub. It's not a great look if the bub's mum won't even access basic support programs for her daughter, leaving all financial and practical support coming from the grandparents.

Alex has a swallowing reflex. Hers is just poor. Some might go into her respiratory tract rather than GI. Drooling is a better outcome.

I would 100% believe that Noah’s parents and Alex are both the type of idiots who insist they are self-reliant bootstrappers who would NEVER take a gubbmint handout like the blacks and Mexicans, all while blissfully cashing in on Medicare/aid, ssi, homeowner tax credits, ppe loans, federal/state student loans, etc.
