ZERO DOCTORS (the ones with ethics and professionalism) will approve of Hambutt getting surgery--or even WASTE TIME in an exam room with her. Hambutt is a different case than Tammy, but not by much if we're talking about ethics and professionalism. No doctor who cares about their patient outcomes or their very license, will so much as entertain Amber even speaking about WLS.

Tammy has indeed had bariatric surgery. The surgeon was the same surgeon Amber is now seeing…Dr Eric Smith of Kentucky Bariatric Institute…not Proctor. I have no doubt that is why Amber chose him specifically given she had other options in Lexington. If he would perform surgery on Tammy and her brother, then Amber would expect she is even more likely to be approved than Tammy.

As for the ethics…sure it’s questionable. However, Tammy has already had at least three lucky escapes from death in the past 2-3 years. This surgery has the potential to save her life, whether it does or not doesn’t really matter, because she’s going to die anyway. I’m sure that both TLC and the surgeon would frame this surgery as giving her a chance to turn her life around when there is no other chance.

Amber on the other hand, doesn’t appear to be at that point yet…plus she’s not being filmed by TLC cameras (as far as we know). The surgeon’s expectations of Amber may be much higher as a result. We need to remember that Amber, by her own admission, has not even met with the surgeon one on one yet. Her only experience of him so far is through watching him on a contrived, heavily edited, and chronologically screwed up TLC show, and his brief chat to a group of potential patients at an information session. He is likely a very different beast in the presence of a 500lb plus tub of lard in the privacy of his consulting room.

No idea how water weight varies for someone of her impressive girth, but I'd absolutely believe she can swing 20lbs on a scale in a single day.

She's consistently been in this same weight range for years now, so I suspect her body has adjusted to the lifestyle, and won't change barring a sizeable/sustained caloric increase/decrease. A moderate and sustainable calorie deficit won't do shit at this point.

@Boolean Bitch, Of course Amber isn’t going to tell us which surgery she wants, because she’s unlikely to get it. She needs to pay more attention to her TLC shows. Amber has stated very clearly in the past that she wants Gastric Bypass, but all these TLC super fats are getting robotic Sleeve Gastrectomies…for good reason. Chris Combs and Tammy Slaton (not yet officially confirmed but widely rumoured it was Sleeve) were both Sleeve Gastrecomies, all Dr Now’s 600lb lifers have been getting robotic sleeve Gastrectomies recently. The only recent TLC surgeon gastric bypass that’s been televised in the last year that I can recall is Vanessa from 1000lb Best Friends, and she weighed significantly less (398lbs) than Amber at the time she was approved for surgery.

Maybe being prepared to pay more out of pocket might get Amber Gastric Bypass instead, but guaranteed she’s not going to consciously disclose which surgery she wants until her surgeon confirms he’ll perform it on her. I suspect Amber has had too few interactions with specialists and surgeons to realise yet that when they tell patients they have options/choices, it’s the surgeon that makes the ultimate decision, based on patient circumstance but also risk of complications that in the long run can damage both the surgeon’s reputation and increase their already very costly insurance premiums.

Bypass has fallen out of favor which is why it’s really no longer recommended. Gastric sleeve is safer and has less risks, though both come with dumping syndrome as a consequence as stated before.

But here’s why a surgeon may choose gastric sleeve:

gastric bypass surgery poses a 4-7% risk of developing an ulcer, whereas gastric sleeve patients don’t ulcer. Additionally, there is a 4-7% chance of a gastric bypass patient needing a follow-up laparoscopy, where as a bypass will usually cause t2 diabetes remission.

I’m not saying they are never done anymore, but bypass isn’t as common as it used to be. Also think of staples and the kinds of issues those could cause.

Gastric bypass is still the gold standard WLS. It is however a longer and technically more difficult surgery and therefore less safe for the super duper morbidly obese. It's also a cheaper surgery so favoured by those who are self pay. The VSG is a shorter surgery that involves no digestive system reorganisation. The VSG is preferred for the ultra fats because it is far quicker to perform and they can get pretty good results to a point. When they are smaller and have shown they have made the necessary lifestyle changes, and less of an anesthesia risk, a second stage surgery can be performed to convert the VSG to a DS (Duodenal Switch).

VSG rarely comes with dumping syndrome, and questioning of patients who claim to have it usually confirms that they don't. What VSG does come with is a very high risk of acid reflux. As the VSG has no malabsorption component it is often recommended for younger females planning future pregnancies, but that still doesn’t prevent gastric bypass procedures being approved for those women. Gastric bypass is well known for better weightloss results in diet compliant patients, and those results are backed up by decades of studies.

But here’s why a surgeon may choose gastric sleeve:

gastric bypass surgery poses a 4-7% risk of developing an ulcer, whereas gastric sleeve patients don’t ulcer.

They do still get ulcers along the stapleline, although it is not as common. VSG patients will have far more issues with acid reflux though, and in patients with pre-existing GERD, a gastric bypass (or variant) should be the surgery offered as it is the cure for GERD, whereas VSG will exacerbate that health issue.

Additionally, there is a 4-7% chance of a gastric bypass patient needing a follow-up laparoscopy, where as a bypass will usually cause t2 diabetes remission.

That's a new one to me. I'd appreciate seeing a source study for my own educational purposes. The laparotomy part that is. The bypass diabetes stuff is common knowledge. VSG patients are far more likely to need revisional surgery which is both more complicated and has an up to 20x higher risk of very dangerous staple line leaks.

Also, VSG is NOT reversible as the remnant stomach is removed. Technically a gastric bypass IS reversible as the remnant stomach is still living in the abdominal cavity. Reversals are rarely ever performed though. Also, after bypass, the tissue in the remnant stomach is available to be used should it be required later in life to repair the stomach eg. If cancer needs to be removed.

I’m not saying they are never done anymore, but bypass isn’t as common as it used to be. Also think of staples and the kinds of issues those could cause.

The VSG uses far more staples than a gastric bypass. In the VSG the stapleline runs the entire length of the stomach, whereas in a gastric bypass the staples only run across the much narrower top of the stomach. If you think there are no staples in the VSG, then you've just proven your ignorance of WLS procedures.

If you are instead referring to Endoscopic Sleeve Gastrectomy it is NOT the same thing as a VSG, is NOT a common or recommended procedure, and has very poor results and poor durability as the stitches breakdown over time.

Now this won't happen. The only way any fat is being carved off off her is if she has loose skin removed. While I do think she'll eventually qualify for WLS...not forgetting she still has to find a way to pay for it...I don't believe she'll ever lose enough weight for a plastic surgeon to approve her for skin removal surgery.
