I really don't want to think about this too hard, but I think the rash is more likely caused by friction, sweat and poor hygiene. It's quite common among larger folk, especially gentlemen, as that area doesn't have great air circulation and has a lot of crevices. Which means that having a plastic strap rubbing against it all day is going to hurt like hell, and he may wind up with another infection.
