I'd wonder how much of this is down to his lungs being fucked by being a pre-me and then lunging tobacco in all known forms nearly constantly than something like sleep apnea, I think that he sleeps in his collar or at least passes out in it often enough to have an effect on his health.

Early sleep apnea (like before your 40) can cause a whole host of other medical issues and cause others to show up decades earlier than they otherwise would and may have a link to dementia, combine that with his poor health in general and add into that his alcoholism, heroic tobacco and other substance use he's really heading for a massive implosion in his health in the coming years.

Try to think of it like this, right.

There's a reason why he's afraid to talk to women and engage with them romantically. It's because, despite his delusions, he knows exactly who and what he is. This often comes out when he's emotionally compromised, he'll call himself all the things the trolls call him. If you gave him a win, yes, he would go on camera and say "Dood I tawld u guys, just gotta wait patiently and rock the tacticle sope." This would however run contrary to his internal dialogue, which is usually "You're never getting laid again because you look like a creepy swamp creature pedophile." That might do him some good, not hating himself so much.

I don't know if it would work and frankly I don't know how effective it would be if it did.
