Lecter was a psychiatrist for Verger who got him drugged out of his mind and then talked him into cutting his face up to feed to dogs. He was already a freak anyway but he was off his rocker when he did it.

It was more based than that. He was a patient of Dr. Lecter. He was also guilty of touching young boys.This upset lecter who more so in the books put a premium on civility and righteousness decided to correct this wrong. He drugged verger during a session and got him to cut his face off with a broken mirror. There is more nuance in the book especially behind this event but its been so long since reading the books. It also didnt help the movies dont actually follow the actual time line of events chronologically. The books however when read in order I highly recommend.
