Either chronic kidney infections or a single big infection that goes septic can do serious damage, and Gruffin is prone to BOTH. If she lives to an old age ( ), it is highly likely that she will experience renal failure and need a kidney transplant (I cannot imagine her being a good candidate for transplant) or dialysis. Tbh if her kidneys are still functioning in 20 years I will be impressed.

This happens because actual female puberty is a lot more than what happens when you give a male estrogen. Other hormones like progesterone contribute to the growth of supportive structures (even if these men take progesterone when their moobs are already like this, it’s probably too late, you need it at the right moments). We have actual mammary glands that mature. There are probably genetic differences from the sex chromosomes. It’s like how women taking testosterone do get a lowered voice but it often doesn’t sound like an actual man. Sometimes it results in that frog voice, the TiF equivalent of the tuberous moobs. The underlying structures are different and no amount of exogenous hormones can change that.

A few reasons. The female bladder isn’t as strong as the male so it can’t efficiently push out urine over a longer distance. The extension creates a bend that doesn’t exist in the male urethra where urine can stagnate. The surgery creates strictures and fistulas so that leads to festering urine too. Well, I got chronic pyelonephritis from this, but thankfully in my adult years it's in long remission now. I'll never forgive nor understand why did they do this to kids though.

I can't speak with regard to private K-12 but my experience with the public ones is that the teachers go on major power trips about restricting access to the bathroom and it causes children a lot of problems. It was a multi viscera transplant and she was hoping for organs specifically from a dead child because she was anorexic and wanted to be smol enough to need child organs. All the organ damage was her own fault. She should have been on hospice.

that is fucking brutal, just horrible. i hope it didn't cause the UTI to progress further, i hope they didn't consign someone to a life of disability cos of their dumb rules.

pyelonephritis can fuck you over without prior symptoms of UTI. it happened to me, even my pee came back clear (i'd been drinking a fuckton since starting to suspect a kidney infection, might have caused it?), but the GP sent me to A&E cos i had rigors and thank fuck he did.

that was uncomplicated pyelonephritis and it was absolutely brutal. these pooners have complicated their urinary systems to fuck in ways i doubt are described in medical textbooks. like what is the urologist at a provincial hospital supposed to know about this stuff.

what i don't understand, and perhaps a medfag can explain, is why, with their lengthened 'urethras' they are so much more prone to them. given women get them more than men cos our urethral openings are closer to the anus, why isn't increasing the distance reducing their risk? i'm guessing its cos bacteria can get in where the flesh tube is sewn on, or one of the random places pee comes out when they get strictures. so then are abdo phallos or ones with no urethral lengthening lower risk compared to the others?

Others have explained very well, but also keep in mind that in the typical phalloplasty, the "urethra" is made of sewed-on arm skin. The male urethra is specially set up to get pee out effectively. Rolled-up arm skin isn't.

Another issue is a lot of them have trouble fully emptying their bladders due to the surgical fuckery. Some have said that it takes them minutes on end to pee. So they drink less, which makes UTIs more likely.

Plus, even without phalloplasty, stopping all female hormones causes a certain degree of atrophy in the female genital area, including the entrance to the urethra. The vaginal microbiome seems to be dependent on the healthy cells there getting estrogen, because without it the pH changes and the healthy bacteria start dying off, leading to more infections from bad bacteria. This has mainly been studied in regards to post-menopausal women. It can be treated with estrogen cream, but I don't know how you would apply that when what was formerly the entrance to your urethra is now buried under a fleshy Coke can of arm skin.

UTI is the same even in cats — you feel urge to pee way more often but then you try to do the thing you either pee very little or can’t at all, but at that moment your whole pelvis is struck with pain, like your small muscles around, (forgive me Thomas,) urine-related organs, spasm simultaneously.

So I can not imagine how can they not notice having uti.

Given what some have said about their issues with peeing, the feeling of urge to pee constantly and the sensation of not emptying the bladder would not be that unusual. And all their nerve signals from the area must be fucked up from the constant abuse, long-term catheter use, etc. Their brains probably barely recognize what they are feeling anymore.
