Wait, rib removal is a real thing and not something that gets done in playground rumors? Huh.
The last 2 pairs of ribs do not attach to the sternum, and are "floaters" that can be removed without too many problems.

It is a rare elective surgery for those professionally working the pageant circuit, but more commonly, people in extremely traumatic accidents just get these ribs taken out because there is no real way to brace a rib.

Can any of them explain what makes them different from any plastic surgery addict? The ones that want to turn themselves into cats or Ken dolls? Anybody getting a hit of their addiction will feel "euphoria," that means nothing.
The difference used to be, traditionally those suffering from gender dysphoria are doing this to stop feeling a constant sense of what could be best described as nausea. For some, it really does work.

Today, we don't actually care if the troon is actually mentally ill.

Does anyone with more knowledge on the general body modification scene (including those Barbie/Ken people) know if it’s as predatory as the SRS surgery circles seem to be? I always got the impression they just quietly did their own weird shit without trying to talk everyone else into doing it too, but I could be wrong :\

Generally, you need legal standing to have "surgery". This could be as simple as signing off on therapy sessions and signing a waiver, or as complicated as suing the govt for permission to go ham on your body.

Extreme bodymodding often is in a legal grey area, because most legislatures have a rather tautological definition for a surgical procedure, which amounts to "if a medical practitioner calls it a surgery then it is".

As a result of this, the extreme body mod crowd goes underground to get their fix, and accordingly develop their own networks and cultures that have peer pressure and one-upsmanship.

Sometimes a surgeon starts offering a bodymod procedure, retroactively making it illegal and suddenly you see a string of busts in the scene for "practising medicine without a licence", and you can bet there was some political fallout in the community.

Other times, the surgeon gets asked to do something but the surgeon refuses due to insurance implications. Sometimes you see a surgeon reprimanded for doing things like earlobe piercing during surgery.

All I can think of with the leg shortening/lengthening nonsense: If you’re so tall or short that a few inches difference won’t help, why bother? And if you’re taller/shorter than average for your transitioned gender by just a bit, wouldn’t it be “close enough” to bother with a very intensive surgery? I know that many will still do it anyway regardless but it’s just not worth it.

This surgery is for children with polio whose legs became deformed, uneven in length or brittle. Since the legs have to be reinforced anyway, about ~15cm of height is added in up to 5 successive surgeries to match the patient's projected growth.

Polio is currently considered eradicated. Naturally, these surgical practices are now only seen in Asian nations where leg deformities are more common.

Incidentally, if you have leg alignment issues and you happen to break your leg, it will be straightened. What often happens in this case is the other leg is broken and re-set at the same time so you don't walk out with one straight leg and one crooked leg.

Also, orthopaedic surgery can only be performed by orthopaedic surgeons, so it is strange that these cosmetic surgeons are so willing to have their teeth kicked in by advertising these things.

I have a clinically retarded question but I have to ask:

Why can't they harvest stem cells into new skin grafts for them? Why do they still resort to flaying them alive using the skinning-a-chicken method?

Because stem cells are genetically male or female, and are grafted onto surrounding cells which are genetically male or female and already have hardcoded information on what cell they are meant to be.

In short, there is nowhere for stem cells to get the information that they are meant to become a neophallus or neovagina.

Also, stem cells are applied in a suspension that is a very thin film layer, about the thickness of single ply tissue. It would take ages to grow a usable sample for such a purpose.

This is a brief overview of the first 7 1/2 months of recovery of my GCS surgery. It's not pretty, but they rarely are. OK, I know this is hard for some people to view, but this is the reality of complications and their impact.

This page hasn't been updated with the story of my first 4 days in the hospital. That deserves its own separate story altogether because it was a nightmare complete nightmare on an entirely new level. I communicated the entire ordeal in detail to Dr. Gallagher, but the best she could muster in response was;

"I'm sorry that happened to you."

I'm sure you can all see what she did there in that statement. This is the way surgeons speak when they don't want to take any responsibility for what they did. There's a lot of that misdirection if you pay attention. They do it pretty consistently.

The swelling seems a bit much and I'm concerned about the suture lines at the base after 12 days. I'm taking it very easy and there's no pain or discomfort. Bladder control is increasing slowly. Hi, Everything still looks fine - the yellow areas are what we call exudate and there may still be some separation just at those areas. This is still looking like the classic slow perineal wound healing a lot of girls get. It is probably going to look worse before it looks better and you will likely see loose sutures. The good news is it will scar very well and it doesn't look infected. Let's keep tabs on it and we can decide whether or not I need to see you Wednesday. I'm glad the bladder is improving. Keep in touch I appreciate the updates.

Try not to worry about that- IF we do have to do anything it will be very straightforward nothing at all like a recovery you are going through- it will be a case of a few dissolvable stitches and minimal downtime. My money is still on this area surprising you and healing in on its own -the time frame can be as little as 2 weeks.

Late MAY, Early JUNE 2017, and I started to see this red tissue that I believed was granulation build up at the vulva entrance. The incision would tear along the inside corners when dilating. You couldn't see any of this from the outside, but it kept getting worse. But OK, I should listen to my Doctor. Right?

So that looks more like exudate to me just likely a small area of graft the didn't take and is still healing in. Granulation tissue is more pink and grows out and bleeds a lot. The white stuff eventually goes away on its own. I'm not seeing any granulation tissue on you. Happy Memorial Day

AUGUST 22nd & 24th, 2017 -- How much did this persistent hypergranulation hurt?
Tears, and a lot of them. Dreadfully painful. Dilating is almost impossible because of the pain. Can't sit in a desk chair for more than an hour with a cushion :(
This is where everything is really starting to fall apart. The pain is just terrible and it's not getting better it's getting worse. I'm seeing the Physical Therapist who I really like, but we aren't doing any work on pelvic floor issues because the problems are all at the Vulva entrance. This has been my main concern from the beginning. I don't know why they kept insisting this was a pelvic floor issue that what causing this problem.
We worked on trying to massage the tissue to increase the vulva opening for weeks until I was was a mess both mentally and physically. I thought the first 6 weeks were bad enough but at this stage, it was nothing but torture.
I reverted to the smallest dilator in order to maintain depth. We tried using a softer silicone dilator with some success, although even that because too much after a while. Eventually, it was all too much and dilation turned into a desperate attempt to hold on until it could be addressed surgically.
The jiffy part of this downhill slide was that it lasted about 10 weeks. From roughly mid-July, 2017 until October. Some days were better than others. I could get around alright, do short walks, but it felt about how it looks in the pictures and it wore me down more and more every single day.
You tell me. Does this look like a pelvic floor problem?

OCTOBER 6, 2017 -- Appointment for Revisions
Both my wife and my Physical Therapist attended this appointment with Dr. Gallaghr so everyone would on the same page in terms of the treatment plan. I made it crystal clear to everyone several times, that whatever needed to be fixed, I wanted it done during this surgery. I DO NOT WANT TO HAVE ANY MORE SURGERY. I could not make this any more clear. Dr. Gallagher agreed that a revision was warranted.
My three concerns were repairing the Vulva and removing all the scar tissue, removing the excess labia tissue and reducing the prominence of the clitoris. The Date was set, but we still had to finalize the details in another appointment.

This one really is uniquely bad even among the other examples we’ve seen. I think I can agree with it being the #1 worst. From the terribly done sutures on the side to the fact that the surgeon made no attempt to even TRY to be symmetrical. To the unique gaze INTO a spread-open stinkditch, revealing the horrors within.

I really don’t think there can be a better example.

I...don’t even know what to say. I read everything and I’m horrified. The poor man. I know he did this to himself and play stupid games, win stupid prizes but he’s definitely mentally ill and a doctor destroyed his life while the community cheered. It’s so horrifyingly sad but also fascinating.

[From A Dilator for Difficult Granulation]

GRANULATION: When your dilator feels like it's made from broken glass.

Granulation Is a very common complication during recovery from GCS. Especially if your incisions dehisce. In case you are unfamiliar with wound dehiscence, it's basically when your incisions come apart leaving an open wound. In other words, your stitches all come apart. In this example, The world-renowned gender surgeon declined to intervene in the matter and simply explained that it would all come together and heal on its own through a healing process called Second Intention. I listened to my surgeon of course, but in hindsight, If I had anything to say about healing through Second Intention it would be more aptly named:

The Long Torturous Road to Healing with Granulation.

It started with the wound dehiscence from my GRS after about 9 days in. (Of course, I was freaking out!) I was told by my Surgeon that we would have to let it heal on its own through "Second Intention". Just an FYI, If your Surgeon ever tells you that your dehisced wound will have to heal through Second Intention, the first thing you should know is:
The chances are, at a minimum, you just doubled your wound healing time. Keep that in mind.

As no surprise to anyone, this chain reaction of pain and suffering caused my vulva to heal improperly. However, I was assured this could be addressed in a revision seven months out. As you can see in the images below, this healing through Second Intention caused a restriction at my vulva entrance. Dilation turned into a nightmare while trying to maintain any size and depth. The more effort I made, the more scar tissue and granulation developed. The more painful it all seemed to become.

June 11th, 2017 - 14th week
Eventually, sitting, walking, or doing much of anything longer than 15 minutes, except laying down, was extremely painful. The largest Soulsorce dilator I could use was the small orange one. Using silver nitrate only caused more tissue buildup from scarring and tearing.

Needless to say, the granulation came roaring back in August 2017. I was working with my Pelvic floor PT, but the dilation became so painful the only thing we could work on was maintaining my vulva opening. It was so painful, I broke down about every appointment while my PT, slowly massaged and stretched the ring of white scar tissue around the entrance and pushed through the granulation tissue.

Switching to The Soft Dilator
Unfortunately, the dilation continued to be painful enough it frequently brought me to tears. There were many times I felt like giving up. My Physical Therapist suggested we look for a softer silicone dilator and see if that would reduce the pain enough to keep it going. You can get them in various sizes they are just cheap soft silicone Adult sex toys. The one I found that worked the best was less than $12.00 USD on Amazon.
It was significantly more comfortable against the granulation.

One more tip that helped control the pain along with the softer dilator. I wish I had used this sooner.
Lidocaine
You can buy water-based lubes, creams, and sprays online that contain as much as 5% Lidocaine. This does wonders to reduce the granulation pain. Just remember, you can't overdo it with the dilation when using any numbing agent. You might cause more damage. I used this solely to tolerate the pain from the granulation. A little dab of this lidocaine and lubrication, while still using Surgilube as the main lubrication, went a long way to dull the sharpness of the pain.

October 27th, 2017 - 30th week
By October and 7-months into my recovery, the granulation in my vulva started to subside. It was still loaded with scar tissue that reduced my vulva entrance to the bare minimum to use the purple dilator in order to maintain the depth. All the scar tissue would be removed in a vulvectomy during my revision planned for mid-November.

There's not a lot of information on her from before she started doing tranny surgeries, at least not that I could find.

She apparently did train as a plastic surgeon before starting her tranny clinic in 2015 but I couldn't find any reviews or patient's pictures of her work. There are two photos of regular breast reductions and one of implants on her site, but that's it. The rest is all troon stuff and stories about her start around 2018.
 
Because I was curious as to what a W-plasty was, it’s apparently a scar removal technique where the scar is cut out with a zig-zag contour and sewn back together, which apparently results in a less visible scar due to the way skin heals:

Post-revision surgery #3, Feb 5th, 2021- Dr. Cecile Ferrando, The Cleveland Clinic

April 17th, 2021 - Updates 10 weeks post-op
I am extremely pleased with this revision outcome by Dr. Ferrando. This is the 3rd revision of my original GRS in March of 2017 and the FIRST surgery that did not end with wound dehiscence or additional complications.
This surgery included the removal of a deep vulvar inclusion cyst at the top of my vulva. This area has been a scarred-up mess for more than two years before the latest revision with Dr. Ferrando. Scroll down to follow the rest of the story.

Updates
Dr. Ferrando is only responsible for the revision in the Clitoral hood area to date. I'm hoping she can fix the shortened labia to extend down and around my vulva. We will discuss this next month. I'm thrilled with her revision.

Post-revision +19 Removal of Vulvar Inclusion Cyst
This is the best my vulva has looked since Dr. Sidhbh Gallagher surprised me by doing a W-plasty at the anterior of my vulva that turned my lower mons area into a carnival of scar tissue. After 3-years of suffering from the continuous rupture of two large inclusions cysts that formed in my incisions. I finally feel better about now that I won’t have a mass of scar tissue be the first thing I see every day in the mirror.
It doesn’t change the fact I have to take two medications to urinate and control my bladder.
It won’t alleviate the nerve damage I have around my bladder and Pelvic floor.
It won’t repair the hideous vulva Dr. Gallagher fashioned out of a flap of my labia.
It won’t give me back the $4200.00 I paid in deductibles for yet another revision.
At least I can feel good about this one.

I like the way this is healing and I feel like the danger of wound dehiscence has passed. None of the cyst removal revisions were done by Dr. Gallagher.

The latest pictures below are from the February 05, 2021 revision, except for the first two at the top.

The latest surgery I had was to remove the final Vulvar Cyst left by Dr. Gallagher's first revision on Nov 16th, 2017. Hopefully, this will end the three-year nightmare of living with these cursed pits of pus. You can see the incision at the top from the last one removed. It was 3cm deep. There's still a suture running through the length of it.

As far as my experience with Dr. Gallagher is concerned, it has been a lesson in surgery complications from the initial surgery and the worst 4-days in a hospital that I have ever spent, until now --as in today. Four years later, I'm still coping with permanent surgery complications that require ongoing medical intervention at my expense and time, this says nothing of the debilitating depression I had to wad through before I could function again. If you believe for one second that Gallagher has taken any responsibility for this disaster, you would be gravely mistaken. This surgeon, Dr. Sidhbh Gallagher, did nothing but make my surgery experience worse at every turn. I know. I was there for every painful moment as she and her employer and their army of attorneys covered up the mess. All I wanted was an explanation and an apology. That would have been the right thing to do for her patient. Instead, she traded it all in to protect her reputation until the bitter end. Don't be fooled by her chicken-wing yacht-dance and her big bottle of champaign.

We, as Trans-people, are just a means to an end for her. I had nothing but the highest regard and respect for Dr. Gallagher in the beginning. I let her gaslight me into complacency knowing that she was the only one that could correct the very problems she created. In time, I discovered other problems that forced me to walk away from her practice altogether. Problems that couldn't be corrected by a scalpel and a smile. The retaliation From Dr. Gallagher and my healthcare provider started 7 months later. I'm still paying for her mistakes 4 years after my initial surgery and I have all the scars and wounds across every part of my life to prove it.

So, ya. If you are looking for a surgeon who will uphold their oath of "Do no harm" and does well preparing you for the surgical reality you may face under her scalpal, Dr. Sidhbh T Gallagher may not be the most reliable choice for your gender affirmation surgery. As a self-described "Renowned Plastic Surgeon", she might want to take a peek over her shoulder and gander at the carnage and disappointment she's left in her wake. She can look me square in the face anytime she's ready and explains herself. That's all I have ever asked for.

I implore Trans-people who are considering surgery with Dr. Gallagher. You have one shot at this. Don't let her do to you, what she did to me. I can assure you, my experience is not unique. Four years after my surgery, all I have to look forward to are more revisions, financial cost, and medication as a direct result of choosing an Irish accent over a competent and experienced surgeon who isn't constantly making the same mistakes. It was a terrible choice and an even worse outcome that I'll never fully recover. This is a club you don't want to join.

This is how the whole cyst problem started with Gallagher's first revision surgery on Nov 16th, 2017 Those two blue fingers are Dr. Gallagher's showing me and my wife what the results of the revision were going to look like. She took this, and other pictures like it, with my camera. At no time does she explain the W-plasty she is going to do.

These images are from the latest surgery on February 05, 2021. The goal was to remove the final Vulvar Cyst buried at the apex of my vulva - I am extremely happy with this outcome. Some, but not all of the scar tissue was removed and the surgeon was able to fold the labia together at the incision. NO VICRYL SUTURES were used. There is a single dissolving suture running through the incision. The best part has been the absence of any wound dehiscence, which has become my biggest fear in these later surgeries.

It took months for the May 22nd, 2020 wound to heal through Second Intention. The other cyst started the same routine of filling up and breaking open. Again I'm back to finding another plastic surgeon to remove the cyst.
If you notice where the cysts are located, they are right above the nerves leading to the clitoris. Any access to this area for intimacy causes friction around the cyst causing inflammation and eventual rupture. There is no escape from these cysts now for over three years now. Not a single word of apology from Dr. Sidhbh Gallagher.

In addition, I lack all but a few small random spots of tactile sensation in my labia and vaginal canal which I have been advised by 3 different Urologist/GRS surgeons, all at different hospitals, that the nerve damage is most likely permanent. Surprisingly enough I don't recall seeing Permanent Nerve Damage on her non-existent list of possible complications.

These images below were taken AFTER the Removal of the first large inclusion cyst on May 22nd, 2020 Everything started out good and then, like clockwork, the wound dehisced about a week in.

A few of this guy's pictures have been floating around awhile, but I think his full story & photo albums are new (AFAIK). It's pretty informative, at least IMO. Wish I could just archive all this and post links, but archiving sites never seem to work right on image-heavy pages.
I'm sorry for the massive posts, though. It's definitely getting to be a bit much... Won't be doing something like this again (not worth the effort in hindsight, lol), but I feel committed to finishing this now. 😅

As terrible as surgeons who do this shit are, you willingly and knowingly walked into a 100% cosmetic procedure with zero research on the possible consequences outside the troon hugbox.

Alves says that before the surgery he has never seen a vagina in his life or have been with a woman, though according to the nurses in the hospital his vagina is really beautiful. He himself says "my vagina looks beautiful, it is a work of art" and at the final appointment where he talks with the butcher he says "it's a little bit swollen, but it's good and my labia is very sensitive".

I'd be intrigued to see how it actually looks like, but considering his poor judgement and how most of these surgeries turn out I am very doubtful it is one of those best ones out there.

He is now considering a voice feminization surgery – which should not really surprise me at this point is also a type of surgery that even exists, yet it did – "in order to make my voice 100% feminine". This is what made me post it here:

Surgeries for voice feminization focus on raising habitual speaking pitch by reducing the ability to produce a low-pitched voice. This means that surgery will reduce the overall pitch range of your voice. There is also a risk that surgery could cause your voice to become too high or so rough, hoarse, strained or breathy (dysphonic) as to make communication difficult. Because voice feminization surgery will only change your pitch, you may still need to work on other vocal behaviors.
Two surgical options used most frequently to raise voice pitch include:

  • Anterior glottal web formation. This surgery creates a web or scar band at the front of the V of the vocal cords (anterior commissure). This shortens the vocal cords to help raise voice pitch. Anterior glottal web formation affects frequency range by eliminating the ability to produce lower pitches. It also narrows the airway to some degree. As a result, this surgery might be less appropriate for vocal professionals and, perhaps, serious athletes.
  • Cricothyroid approximation (CTA). This surgery fixes the cricoid cartilage to the thyroid cartilage, elongating the vocal cords. This results in a higher speaking pitch and a reduction in the ability to lower pitch. However, studies have found that this technique doesn't have a lasting effect.

No one sane would say this. And your nurses definitely didn't say that. Penises and vaginas aren't "beautiful", much less a facsimile of them.
Funny how they call him a virgin and say he's "never seen a vagina" (Homophobic HSTS) yet has some authority on what a "beautiful vagina" allegedly looks like.

LOL, "Transition". Is it a transition if you still look like a man with terrible plastic surgery? Just watched the video, and it's funny about all the terrible looks he got on the street like everyone can still tell, but he claims that he has a woman's face, body, and vagina......
