If I was a trained doctor, with a full set up ready to go, in the middle of the ocean, and someone showed up needing me to pull a splinter out of their hand or they die, and I don't? And they die? I'm totally off the hook. I have 0 responsibility whatsoever, regardless of my training, to provide medical services.

Not so at an emergency room.
