WHO declares highest alert over monkeypox

The monkeypox outbreak has been declared a global health emergency by the World Health Organization.

The classification is the highest alert that the WHO can issue and follows a worldwide upsurge in cases.

It came at the end of the second meeting of the WHO's emergency committee on the virus.

More than 16,000 cases have now been reported from 75 countries, said WHO director general Dr Tedros Adhanom Ghebreyesus.

There had been five deaths so far as a result of the outbreak, he added.

There are only two other such health emergencies at present - the coronavirus pandemic and the continuing effort to eradicate polio.

Dr Tedros said the emergency committee had been unable to reach a consensus on whether the monkeypox outbreak should be classified as a global health emergency.

However, he said the outbreak had spread around the world rapidly and he had decided that it was indeed of international concern.

"The WHO's assessment is that the risk of monkeypox is moderate globally and in all regions, except in the European region, where we assess the risk as high," he added.

It doesn't work nearly as well as people think it does:

Subjects were followed for 4,237 person-years. The primary outcome measure was the incidence of documented HIV seroconversion. At the end of treatment, emergent HIV-1 seroconversion was observed in 131 subjects, of which 48 occurred in the TRUVADA group and 83 occurred in the placebo group, indicating a 42% (95% CI: 18–60%) reduction in risk. Risk reduction was found to be higher (53%; 95% CI: 34–72%) among subjects who reported previous unprotected anal intercourse (URAI) at screening (732 and 753 subjects reported URAI within the last 12 weeks at screening in the TRUVADA and placebo groups, respectively).

Attached is the prescribing information that says this. It's near the top of page 31 under section 14 Clinical Studies.

The 90%+ figure that Gilead likes to quote is its "Perfect Use" rate. In medicine no one every uses that number because it is essentially useless. No one has perfect adherence. The 42% reduction in risk is what is usually called the "Typical Use" rate, this is how well it really works when "human factors" are involved. The Typical Use rate is what you want to use for how well a given medication really works.

For anyone that wants to say "This is for Truvada PrEP, not the newer Descovy PrEP" Descovy is no different. They used a comparison to Truvada as the basis to show that Discovy was not inferior (it's called a non-inferiority study, you can use it to get a medication approved by showing it doesn't suck any worse than what is currently being used, great standard, right?) as one of their main points. The data for the two was extremely similar.

So, anyone relying on PrEP exclusively to not get HIV, good luck, you're going to need it.
