Remember to look and listen. Is the witness hesitantly answering? Who glances at who and so forth. Remember, a transcript is public, so if you can latch your note to a particular question, you don't have to get every word. Accuracy, not precision.
I think a good way to think about it is that it is not about writing down every word verbatim. The transcript will have that.
But write down enough of what is said so you can correlate to the transcript later and add flavor.
This could be like adding flavor to the transcript "at this point/this question Nick hesitated" and "at this other question he got angry and almost screamed at the judge".
