I'd wager it's because he used to be a fat fuck and never got any skin removal surgery when he lost all that weight. The bathtub hrt is also a possibilty as you pointed out. Certainly isn't helping him not look like a science experiment gone wrong.

He was in Japan and apparently Israel. It's his time in Japan that got him attention. He's currently in South Korea facing jail time. He wanted to got to China and Thailand next. I doubt he'll live long enough to leave S.Korea with all the charges he's facing.
