Regardless of where the pills initially came from, hospitals don't allow unsecured medications to just sit around in a room. Even if it's a medication the patient was taking at home, part of acute care is keeping an accurate log of medications and times to assist in clinical decision-making.

Our only source is biased and trying to gin up outrage. The nurse threatening Risk Management sounds legit if this was a standard "but these are my home meds what do you mean I need a doctor's order to take them inpatient?" modified by it being a big hulking male with emotional issues arguing with staff.

I'd think you'd hold the hormones pre- and post-procedure for a while, but I'm no troonologist. If he were getting e.g. ketorolac while NPO, that's a reason to be concerned about taking bonus ibuprofen.

"Taking away food" and the "food bag" sounds like they were going back and forth on whether he was NPO/had adequate oral intake, and were trying to wean from the NG tube feeding. That could be staff confusion, surgeon's orders changing after reassessment, or patient self-reporting too much pain with swallow, or having adequate PO intake one day but worse the next.

I suspect he wasn't ordered strict NPO because he wasn't posting about thirst, and because of the pills. If he were strict NPO and taking home meds, Based RN would have had more things to yell about.
