a bit late to comment on this, but it could be untreated blockage or clot (maybe) or more likely untreated infection of some kind... feet get a lot of traction in bad places and with the sweat and grim it's a prime place to get an infection of one kind or another.

from how i was trained in the military (and from first aid classes and generally being kind of a outdoorsman) frostbite where you get nipped with redness and numbness is generally from exposure over time and heals with warmth, massage, and sometimes painkillers after a week or so, sometimes longer depending on the body part because the freezing wasn't deep enough to really hurt anything permanently if it did get deep to where tissues like fat and blood vessels froze and blistered, it'd be super bruised for a while but still heal painfully after a month or two. was he walking around in near zero temperatures without a shoe/sock? Lucas doesn't strike me as someone that hard up that he would be barefoot in the cold.

also it affecting one foot is strange, as it implies his other foot/leg did not have the same frostnipping experience.

regardless, if the frost damage was severe enough to be lasting from over a year ago, the foot should be amputated to prevent infection, that implies a super bad frostbite where the tissue has died entirely and cannot heal at all. the blackened dead flesh will turn necrotic and gangrenous pretty quickly once moisture and heat are applied.

While this wasn't necessarily wrong at one point, as he had type 2 and HAD been regularly testing his blood sugar and using insulin for awhile, when he lost a couple hundred pounds years ago and cleaned up his diet, he stopped needing the insulin. That's not infrequent for type 2 as a lot of type 2 is caused by being obese and having a shit, usually high carb (mostly high sugar) diet. Trouble is it's not a cure, it's what more or less amounts to something closer to remission and if you fall off the wagon as he did and go back to your old eating habits and gain the weight back the type 2 often comes back along with it.

But, because Lucas took, "Hey, your last few blood panels indicate that you maybe can go off of the insulin, but keep checking your blood sugar, watch your diet, and watch your weight" to mean, "I'm cured! And cured is forever!" he's back at square one again.

If I recall from some of his much older videos, he fucked up that foot initially with what was probably frostbite (but, then, he's an unreliable narrator; in one video he said he went to the ER after thinking he was going to die of hypothermia on a rainy winter night and one foot has been mostly numb ever since) and is made worse by his untreated diabetes.

This self inflicted gimp walk is one of many reasons he thinks he deserves a girlfriend: He crippled himself for you (somehow), Gen Z!

There are drugs to relieve the nerve pain that comes with it. Lyrica is popular treatment. However addicts buy those to "enhance" their opiate high and I can see Big Wern selling them.
In my state, those are now, along with Neurontin, controlled substances, because of the street use.
If it's controlled in WA, no way they'll prescribe it to a hobo who hangs with addicts.
Overall and sorry to sperg but if he gets a blister from walking in crappy shoes, or calluses, he won't feel them AND given his hygiene, would certainly become infected. Blood flow is restricted to the feet and lower legs with this disease too, adding to infection risk.

Sorry to sperg again but drastic weight loss in a diabetic is a symptom of hyperglycemia (high sugar). The body begins to burn fat and muscle tissue because it cannot use sugar for fuel without insulin or medication. They can eventually go into ketoacidosis and die without intervention.
Side note: If he's on the street I doubt he can store insulin much less use it consistently.

Sorry to sperg again but drastic weight loss in a diabetic is a symptom of hyperglycemia (high sugar). The body begins to burn fat and muscle tissue because it cannot use sugar for fuel without insulin or medication. They can eventually go into ketoacidosis and die without intervention.

His original weight loss, the one where he went from over 400lb down to the mid 200s he actually appeared to have done...normally. Like he cleaned up how he was eating, cut out a lot of junk food (including a fuckton of soda), and started exercising regularly via walking. Hell even if he just cut the liters worth of soda he was apparently drinking at the time out that'd be enough on its own to get him to drop a decent amount of weight just due to the huge calorie deficit it would have caused.

His latest bouts of weight loss, particularly the bit with the LA Catfish saga where he'd occasionally brag about having "lost a ton of weight recently", have been different; those have been rapid and with no change in his diet or really exercise levels. If anything his diet has managed to get worse, and that started while he was still in his little fartbox of an apartment--so he can't blame it on being homeless--because once he heard 'you don't need insulin' and assumed that meant he was cured he went right back to his junk food eating habits despite being able to afford to continue to eat a relatively decent diet.

After he got his ass kicked out on the street again and had to rely on handouts he started the gorging on shitty restaurant food and junk food as 'treats' and got his 'beetus back in close full swing. I'd almost bet half the reason he hasn't gone into ketoacidosis yet is because he doesn't get a chance to be constantly shoving food into his face and when he does it's nowhere near at the level of high sugar junk food he does when he has enough money to afford a motel room for a night or two.

At this point, even if he was willing to admit he needed to go back on insulin, he'd have nowhere to store it and his mental health is so exponentially worse than it was a year and a half ago he'd never remember to check his blood sugar, let alone use any insulin he got. Better to let the insulin go to someone who isn't a worthless pedo leech on society anyway.

I give it a year or so before our first amputation saga. I've said it before, but I feel really badly for the poor medical staff that will have to care for Lucas in the future. I can see a scenario where he's whacked out due to encephalopathy caused by his festering beetus wounds, only for the antibiotics and such to snap him out of it and have him being MORE of a pain in the ass to deal with. At least when someone is objectively not oriented and continually trying to get out of bed, the nurse can dope them up with zyprexa and shit. It's a lot harder to justify sticking him with a tranq dart for ranting about his voodoo jizm to the young nursing aides, demanding ice cream every 30 mins, and insisting that he's just not able to hold his own penis when he takes a leak.

It's so weird how Lucas is able to hold these contradictory positions simply because he feels both sides help his love quest. Like "I'm an anarcho-communist fuck the cops don't be no snitch nigga" and "I am friends with cops and bounty hunters and they're gonna lock up the trolls and if any flatbill disrespects me I'll call the cops on his bitch ass".
