it could very well just be the fact that josh gets anxious in social situations so he drinks a little extra and that combined with his nerves causes him to change the way he speaks. i would not be surprised if he smokes some weed as well especially when NAL is around and as a result becomes even more out of it than he usually is.

it could also be pills but until more actual proof of that is shown i dont believe it. though if anyone was going to be giving him pills it would for sure be NAL.
