As the title says, I have had ten (10) urinary tract infections within the past 14 months. The majority of these ended up becoming kidney infections within 24 hours of the first sign of symptoms. I have been on and off antibiotics constantly and have wound up in the ER because of this many times.

Important context: I am transgender female to male, I haven't had anything done to my reproductive organs or gentials. It's all still female/afab down there. I've been on testosterone for over 4 years now, this means I have gone through male puberty as well as menopause. I work in the sun and drink more water than I can count. I urinate regularly and wipe from front to back. By all acounts, it shouldn't be this bad.

My doctors have done CT scans with contrast and ultrasounds to check my kidneys, they have tried me on stronger antibiotics (as strong as they can provide given my family history of a penicillin allergy (not tested on myself yet but we don't want to risk it)). My doctors have tried putting me on an estrogen pessary to make sure everything down there is working as it should (menopause causes vaginal atrophy and dry walls that can hold onto bacteria and this is supposed to lubricate it all up again), which I am still taking twice weekly and hoping it works. I myself have tried self medicating with Uramet, a daily rUTI preventative as well as another rUTI preventative by Ural. Despite ALL of this, I am once again being hit with another UTI and I am OVER IT.

If ANYONE knows ANYTHING that can help me I beg of you to comment it. I am sick of feeling shit and being on antibiotics. I'm self medicating antibiotics right now with the left over ones from my last UTI to handle the one that's developing right now as I am at work.

Is there a sort of specialist I should demand to be seen by? Is there a surgery I can get to stop this? I cannot keep living like this it feels like hell.
