Is it ego-eccentricity or just cluelessness/one-track mind? Although the fact that he immediately switched back to Kacey argues in your favour. I would be more inclined to let him plead stupidity if he forgot about Kacey entirely and started doing something else when Barb left.
To me the weirder thing is letting Kacey clearly hear Barb without Barb's knowledge. He was a little rude to Kacey granted. But it could be defended a little by pointing out that he may have expected a couple minute conversation and it got out of hand, or that Kacey could have hung up at any time. It seems like a much bigger violation against Barb. If I were having a fairly personal conversation, and it turned out that the person I was talking to was letting someone eavesdrop on every word I would be really outraged. If I were left holding the line a too long I would be a little annoyed.
My guess is that he expected Barb would only take a minute or two of his time, but between his fucked up perception of time and not wanting to "insult" Barb or provoke a reaction from her by telling her to wait a few minutes until he was done with his gf, he decided to just stay there and leave Kacey hanging. At some point he probably forgot Kacey was still on the phone.
